Introduction

The accreditation documents form the basis for a successful accreditation survey. It is strongly recommended that any public health agency considering participation in the accreditation process take the time to thoroughly review the documents.

These documents contain specific information related to the OCCHA principles and standards, the accreditation process, the application form and the agency questionnaire (a self-reporting tool) and the surveyor worksheets (completed by the survey team).

Please note: The Accreditation Documents are protected under copyright law and cannot be used without the expressed consent of the OCCHA Board of Directors.

  1. Application Form

This five page document, signed by both the Chair of the Board of Health and the Medical Officer of Health, indicates the agency’s intent to apply for an accreditation survey. This document includes a brief description of the agency and must be accompanied by the completed Agency Questionnaire. To request a copy of the Application Form, please contact info@occha.org

  1. Accreditation Principles, Standards and Components

This document forms the basis of the accreditation process against which the agency is measured for their degree of compliance. To request a copy of the Accreditation Principles, Standards and Components, please contact info@occha.org

  1. Agency Questionnaire

This self-reporting document is completed by the agency prior to the accreditation survey. The questionnaire is based on the Principles and Standards and poses pertinent questions on the functioning of the Agency. The questionnaire is an important source of information and serves as the basis for the on-site accreditation survey. To request a copy of the Agency Questionnaire, please contact info@occha.org

  1. Health Unit Annual Questionnaire

This document is completed by the agency in the second and third year of their accreditation cycle. This form should be submitted annually to OCCHA on the anniversary of the Health Unit's accreditation survey.  To request a copy of the Health Unit Annual Questionnaire, please contact info@occha.org

  1. Evidence Sheets

This is the definitive report of the accreditation survey and contains all documentation of the on-site survey on which surveyor judgment is based.
